Omgivningsfysiologi

Vid enheten för Omgivningsfysiologi studeras betydelsen av omgivningsfaktorer, såsom gravitation, lufttryck och temperatur, för fysiologiska processer hos människa.

Verksamheter i extrema miljöer

Ämnet omgivningsfysiologi förknippas ofta med verksamheter i extrema miljöer, exempelvis flygning, dykning och polar- eller ökenexpeditioner. Men de fysiologiska funktioner som sätts på prov i sådana verksamheter är i regel av grundläggande betydelse för människan även i triviala och vardagliga sammanhang. I den omgivningsfysiologiska metodiken ingår belastning som ett sätt att få de normala mekanismerna bakom en kroppsfunktion att ge sig tillkänna.

Forskningsverksamhet

Den verksamhet som bedrivs för Omgivningsfysiologi rör i stor utsträckning medicinska och fysiologiska problem förekommande i militära sammanhang och forskargruppen skall vara expertstöd åt försvarsmakten (FM) vid beslut som rör fysisk belastning eller risk för fysisk skada på människa. Forskningen finansieras därför till största delen genom anslag från Försvarsmakten. Verksamheten omfattar tre huvudområden som i sin tur består av ett antal delprojekt:

I vissa sammanhang måste människor inte bara vistas utan även utföra hårt fysiskt arbeta i extrema klimat eller på höga höjder. I projektet studeras fysiologiska effekter av arbete i heta klimat, särskilt hos individer som samtidigt bär tät klädsel och tung utrustning, exempelvis militära soldater eller brandsoldater.

Vidare studeras fysiologiska svar vid central nedkylning (hypotermi) och perifer nedkylning. Av särskilt intresse är att undersöka samverkan mellan olika faktorer som enskilt påverkar risk för lokal eller central nedkylning vid köldprovokation. Exempel på sådana faktorer är syrebrist, fysisk trötthet och sömnbrist.

Barofysiologi handlar om hur människan påverkas av omgivningstrycket. Påtagliga förändringar i omgivningstrycket uppkommer t ex när man förflyttar sig till höga höjder eller vid dykning. All levande vävnad påverkas av hydrostatiska tryckförändringar, men vid de relativt moderata förändringar som åstadkoms vid allt utom extremt djup dykning, är effekterna av det hydrostatiska trycket mindre än effekterna av förändringen av gassammansättningen i lungluften som antingen orsakas av det förändrade omgivningstrycket eller av tillförsel av exotiska andningsgaser. Barofysiologi är därför i mångt och mycket läran om hur olika andningsgaser påverkar människans fysiologi. Eftersom människan inte kan vistas under vatten under mer än ett fåtal minuter utan att hon tillförs andningsgas är en betydande del av forskningen inriktad på hur människans fysiologi samspelar med dykapparater.

Gasblandningar och dekompressionstabeller

Inom projektet Gasblandningar och dekompressionstabeller bedrivs utveckling och utprovning av dekompressionstabeller med andra gaser än luft (nitrox, trimix) för marinens speciella behov. Dessutom genomförs experiment inriktade på att öka förståelsen för mekanismerna bakom dekompressionssjuka (dykarsjuka). Arbetet innefattar både datorsimulering och prov med frivilliga försökspersoner.

Fysiologisk anpassning av dykapparater

Projektet Dykapparater omfattar bl a studier av hur människans andningsfysiologi påverkas av andning med dykapparat, samt hur oxygendosering i slutna dykapparater ska övervakas och testas på säkraste sätt.

Höghöjdsfysiologi och hypoxi

Inom projektet Höghöjdsfysiologi och hypoxi genomförs studier för att utröna kroppsliga och mentala effekter av låga oxygenpartialtryck och vistelse vid hög höjd. Såväl akuta som långtidseffekter undersöks. I projektet undersöks även faktorer som påverkar risken för höghöjdsdekompressionssjuka i samband med flygning med lågt kabintryck.

Forskningen i flygfysiologi omfattar tre delområden: Spatial desorientering (SD), förhöjd tyngdkraft, tyngdlöshet.

Spatial desorientering

Spatial desorientering (rumsliga sinnesvillor) är den viktigaste enskilda förklaringsfaktorn för haverier vid flygning. Spatial desorientering uppkommer eftersom det vid flygning ofta saknas yttre synreferenser samtidigt som innerörats balansorgan inte kan ge adekvat information om vissa rörelsemönster eller tolka förändringar i tyngdkraftens storlek. Vid avdelningen studeras bl.a. hur komplex stimulering av balanssinnet i centrifug och/eller flygplan inverkar på spatial orienteringsförmåga.

Förhöjd tyngdkraft

Vid flygning i högprestandaplan utsätts piloten för tyngdkrafter som kan vara upp till 9 gånger jordens tyngdkraft (9G), vilket leder till stor belastning på flera av kroppens organsystem. Exempelvis, för att upprätthålla adekvat artärtryck i huvudhöjd då man exponeras för 9G i huvud-fot-riktning måste artärtrycket i hjärthöjd höjas till tre gånger det normala. Detta åstadkoms med hjälp av trycksättning av G-byxor och andningsgas men även genom att piloten utför artärtryckshöjande muskelanspänningsmanövrer. Vid enheten studeras fysiologiska effekter av förhöjd G-belastning och utvecklas G-skydds-utrustning/tekniker.

Flera av de fysiologiska omställningar som uppkommer vid vistelse i tyngdlöshet kan framkallas med hjälp av olika simuleringstekniker, av vilka långvarig, kontinuerlig sängbundenhet, i horisontalläge eller med lätt sänkt huvudända, är den vanligaste. Vid avdelningen studeras fysiologiska effekter – kardiovaskulära, muskuloskelettala, metabola - av långvarig sängbundenhet. Experimenten genomförs i regel i multinationella samarbetsprojekt. Kortvarigt kan tyngdlöshet framkallas genom kontrollerat fritt fall i flygplan, s.k. parabolflygning. Omgivningsfysiologigruppen använder även denna teknik för att studera hur kroppsliga funktioner påverkas av tyngdlöshet.

Militär personal utför en mängd olika fysiskt krävande arbetsuppgifter i sitt dagliga arbete. I projektet utvärderas de fysiska och fysiologiska kraven för olika befattningar genom att studera deras arbete i fält. Projektet syftar bland annat till att utforma gränsvärden för fysisk kapacitet hos militär personal i olika krigsbefattningar.

Mycket av den utrustning som används vid avdelningen för omgivningsfysiologi har skapats med anledning av de praktiska problem människan konfronterats med vid flygning och dykning.

Centrifuger

Gondolcentrifugen i Solna, som vid en periferihastighet på c:a 117 km/h genererar 15 gånger jordens tyngdkraft, byggdes med anledning av det stora antal militära flyghaverier som orsakades av medvetandeförlust vid avancerad flygning. Här kunde piloter träna förmågan att upprätthålla blodtrycket i hjärnan vid höga G-tal. Centrifugen används dock huvudsakligen för forskning och teknikutveckling, exempelvis för utveckling av G-skyddsutrustning och för grundfysiologiska studier av bl.a. blodcirkulation, andning (lungfunktion) och balanssinne.

Den omgivningsfysiologiska gruppen bedriver experimentell verksamhet även i den centrifug som finns på Malmslätt utanför Linköping. Med en radie på 9,1 meter (från rotationscentrum till gondolens centrum) och ett maskineri på över 90 ton har denna centrifug kapacitet att på någon sekund accelerera till en periferihastighet på 150 km/h. I gondolen finns en av världens mest avancerade flygsimulatorer, den s.k. Dynamiska flygsimulatorn (DFS), med vars hjälp piloten/försökspersonen kan "flyga" högprestandaplan i virtuell omvärldspresentation och även själv reglera G-kraften i huvud-fot-riktning genom att simulera svängar med olika radie och hastighet.

Den forskning som bedrivs här handlar främst om riskfaktorer vid flygning i militära högprestandaplan och helikoptrar, exempelvis G-inducerad medvetslöshet och rumslig desorientering. Med separata motorer kan gondolen vridas oberoende av den resulterande G-vektorn, vilket innebär unika möjligheter att studera hur människans rumsliga orienteringsförmåga beror av balansorganets olika delar.

Tryckkammare och bassänger

De verktyg som används för att studera effekten av ändrat omgivningstryck är framför allt tryckkamrar. Vid laboratoriet har omgivningsfysiologigruppen både en undertryckskammare där man kan simulera vistelse på hög höjd (>20 000 m.ö.h.) och en övertryckskammare där man kan simulera djupdykning (150 m vattendjup). Gruppen har ett tätt samarbete med svenska marinen och har tillgång till de strömnings- och djupbassänger och den avancerade tryckkammaranläggning som finns vid Marinens Dykeri och Navalmedicinska centrum (DNC) i Karlskrona.
